1. Purpose

Students, student-athletes, coaches, staff members, administrators, volunteers, and other individuals affiliated with Omega Collegiate Academy may have access to information that is private or confidential. By signing this Agreement, the undersigned agrees to protect such information and refrain from unauthorized disclosure.

2. Confidential Information

Confidential information may include, but is not limited to:

• Student and student-athlete records and personal information
• Academic records, grades, transcripts, schedules, and eligibility information
• Medical, insurance, emergency contact, or other protected personal information
• NCAA eligibility and recruiting information
• Recruiting communications and college contacts
• Team rosters, depth charts, playbooks, game plans, strategies, and practice information
• Coaching and staff discussions
• Disciplinary matters and internal investigations
• Financial information, fees, scholarships, fundraising, sponsorships, and donor information
• Academy policies, procedures, internal documents, and business operations
• Passwords, account information, electronic records, and other restricted information
• Private meetings, conversations, emails, text messages, photographs, videos, and recordings
• Information concerning students, parents, employees, coaches, or other Academy personnel that is not intended for public distribution

3. Non-Disclosure Responsibilities

The undersigned agrees not to disclose, distribute, copy, publish, post, photograph, record, transmit, or otherwise share confidential information without authorization from Omega Collegiate Academy.

Confidential information shall not be shared through social media, text messages, group chats, email, websites, interviews, podcasts, videos, or any other public or private communication platform unless specifically authorized.

4. Social Media and Recordings

Students and staff may not post or distribute confidential Academy information on social media. Recording private team meetings, staff meetings, disciplinary meetings, academic meetings, or other confidential conversations without authorization is prohibited.

Nothing in this Agreement is intended to restrict any legally protected right to report suspected unlawful conduct, discrimination, harassment, abuse, safety concerns, or other matters to appropriate government agencies, law enforcement, parents/guardians, legal counsel, or other persons when protected or required by law.

5. Student Privacy

Staff members who have access to student information are expected to maintain student privacy and comply with all applicable Academy policies and federal and state privacy requirements.

Student records and personally identifiable information shall only be accessed, used, or disclosed for legitimate educational, administrative, athletic, or operational purposes as authorized by the Academy and applicable law.

6. Return of Confidential Materials

Upon separation, withdrawal, termination, graduation, or completion of an individual's relationship with Omega Collegiate Academy, all confidential Academy documents, files, equipment, records, passwords, and other materials in the individual's possession must be returned or deleted as directed by the Academy.

7. Continuing Confidentiality

The responsibility to protect confidential information continues after an individual is no longer enrolled, employed, volunteering, coaching, or otherwise affiliated with Omega Collegiate Academy, to the extent permitted by law.

8. Violations

Violations of this Agreement may result in appropriate disciplinary action consistent with Academy policies and applicable law. Depending upon the individual's relationship with the Academy and the seriousness of the violation, consequences may include loss of privileges, removal from activities, disciplinary action, suspension, termination of employment or affiliation, or other appropriate action.

9. Acknowledgment

By signing below, I acknowledge that I have read and understand this Non-Disclosure and Confidentiality Agreement. I understand my responsibility to safeguard confidential information obtained through my affiliation with Omega Collegiate Academy.

I agree to comply with this Agreement and all applicable Omega Collegiate Academy confidentiality and privacy policies.
